GBA new healthcare approaches - FLS‐CARE - Fracture Liaison Service (FLS) to implement an integrated care structure to avoid secondary fractures caused by osteoporosis

The aim of the project is to avoid secondary fractures caused by osteoporosis by setting up an integrated care system that ensures differentiated diagnosis and treatment of osteoporosis.

In the 'FLS-CARE' (Fracture Liaison Services) - project, a network of doctors, nurses and physiotherapists ensures that the diagnosis and treatment of osteoporosis that was started in the hospital is continued during outpatient treatment. A nurse is responsible for coordinating the transfer of patients. Part of the osteoporosis therapy is a fall prevention program including a home visit. A cross-sector IT platform supports the project. Altogether, 1,200 patients who fractured their hip are going to be included at 18 Centers in Bavaria. If successful, 'FLS-CARE' can reduce the frequency of secondary fractures and falls and thus lead to lower mortality and a higher quality of life for patients as well as lower treatment costs. The project is funded with 3.2 million euros from the Innovation fund.
